Medications

When medications need to be given at school, there is a form for both OTC and prescription medications.  Parent/Legal guardian should bring the medication in so that the appropriate form can be filled out and signed.  Prescription medications require a doctor's signature as well as the parent/legal guardian's signature.  This form gives the school permission to give the medication.  Students are not supposed to have any medications with them at any time.   All medications are to be kept in the health room.

 

 When students who take routine medications here at school are running low, I will send home reminder notes.  Some parents prefer to receive a text reminder about the need for a refill.  Please let the school nurse know how you want to be contacted regarding refills.  Please bring a refill of your child’s medication as soon as you can, so that your child does not run out of his/her medication.  

 

Medications must be picked up by a specified date at the end of the school year.   If the medications are not picked up by then, they will destroyed according to district policy.   For safety reasons, we are not able to send ANY medications home with the students.  Parents must pick up the medications.

 

Students that have special dietary needs and changes will need to have a dietary form completed by their family MD.   The form is located below.
